Our clients, who face the end of life, deserve the opportunity to live as fully as possible on their own terms. Patients in need of palliative, home health, or private duty care are also part of our commitment to care. Dealing with short-term or chronic illnesses is a challenge for both the patient and the family. We understand the need to reduce pain and provide the emotional and physical support that is vital our clients and their loved ones. This is what VNA SoCal is all about as we look specifically to the patient’s need to create a care plan that’s right for them.

Founded in 1952, VNA SoCal has always looked for ways to offer more than the expected standard of care. First, we look for solutions to help the patient avoid hospitalizations and feel they are in control of their own care rather than taking a “one size fits all” approach
Our staff is experienced in the unique challenges of hospice care and supporting a patient at home. Additionally, we also have an extended group of volunteers that help their community embrace and support a patient and their families.
As passionate advocates of hospice care and issues surrounding medical decision-making and advanced care planning, we also frequently connect our experts to the community to answer questions and increase awareness of issues that we all need to consider.
We are a community-based nonprofit organization dedicated to providing the highest quality home health, palliative care, hospice and private duty services to patients and their caregivers since 1952.
VNA has been serving the Los Angeles, San Bernardino and Riverside counties for over 65 years. We provide expert, experienced hospice health care as well as offering private duty, in-home services. Our agency also provides 24-hour nursing coverage to support patients and families. We are proud to offer the full continuum of healthcare in the community.

Layne A. Bartholomew is an attorney in private practice in Upland, California. A native of Upland, he has been practicing law since 1995, devoting the majority of his practice to elderly and disabled persons and their families. He is a Certified Specialist in Estate Planning, Trust & Probate Law by the State Bar of California Board of Legal Specialization.
Janet L. Ryerson retired from her independent consulting career in 2000. She assisted US and Canadian organizations in the design and development of custom business applications and accounting software. She currently serves as the CFO for a consulting firm specializing in industrial and commercial development in the City of Los Angeles. Janet is also treasurer for a local nonprofit organization. She served on the City of Rancho Cucamonga Senior Advisory Committee prior to her appointment to the Parks and Recreation Commission in January of 2000. Janet has affiliated with the VNA since 2010 when she became involved with Private Duty Care and then joined the VNA Board of Directors.
