End-of-life decision-making is something we all tend to procrastinate doing. Facing one’s own mortality and talking about it with loved ones is not easy. Many people put it off because they are uninformed or don’t understand all that it entails. In this session, we will cover the basics of Advance Healthcare Directives and the relatively new Physician Orders for Life-Sustaining Treatment (POLST) form that is completed with your doctor to ensure that your wishes are known by your loved ones and honored by medical care providers.
